HIPAA notice

This notice describes how medical information about you may be used and disclosed, and how you can get access to that information. Please review it carefully.

Our duties

We're required by law to protect the privacy of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

What this notice covers

PHI is information that identifies you and relates to your health, your care, or payment for that care.

This notice applies to PHI created or received by the clinicians and covered functions operating through Meto.

How we may use & disclose PHI

For treatment: to provide and coordinate your care, including sharing with clinicians, pharmacies, and laboratories.

For payment: to bill and collect for the services you receive.

For health care operations: quality review, training, and administration of the service.

Other permitted uses & disclosures

We may use or disclose PHI as allowed or required by law — for example, for public health activities, to avert a serious threat to health or safety, and to meet legal or regulatory requirements.

We share PHI with business associates who support us only under written agreements that require them to safeguard it.

Uses that require your authorization

Most uses not described in this notice — including most marketing and any sale of PHI — require your written authorization.

You may revoke an authorization in writing at any time, except where we've already relied on it.

Your rights

You have the right to inspect and get a copy of your PHI, and to ask us to amend it.

You may request an accounting of certain disclosures, request restrictions on uses and disclosures, and ask for confidential communications.

You may get a paper copy of this notice and file a complaint without fear of retaliation.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services.

We will not retaliate against you for filing a complaint.

We may change this notice and make the new notice effective for all PHI we maintain.
